Photo Vista Lens Settings

I just purchased Photo Vista 2.0 and I'm trying to select a lens setting for my
equipment. I have the following hardware.
Sony MVC-FD73 f=4.2 42mm(40-400mm, when converted into a 35mm still camera)
Kenko KUW-042Hi (0.42x w/MACRO Wide Angle Lens)
What are the settings I need to use for this hardware?
Focal Length
Portrait FOV
Landscape FOV
I'm using portrait mode (camera on side - 8 shots) for a 360 panorama.
Once I get this working in Photo Vista, I plan on using this hardware with
Panorama Tools to create a spherical panorama.
