Followup:
Things I have done wrong, repeatedly, in PTGui:
1) Forgot to set Panorama Settings to Equirectangular
2) Forgot to optimize for Field of View before I had properly
characterized my lens.
3) Forgot to save _before_ running the Optimizer and accepting the
results.
4) Pre-rotated my images from portrait before inserting into PTGui
project. If your camera has any 'd' or 'e' offset, it's beter (IME)
to leave the source images unrotated and to let PTGui take care of
the rotation (e.g. by setting all the Image Parameters to -90 degrees
roll)
5) Forgot to set output format properly (e.g. TIFF for enblend, with
a filename ending in ".tif", not ".tiff")
6) Forgot to run APClean and/or clean up obviously "bad" control
points. I once did a handheld pano where the same company graphics
showed up in two places:

Look for the "WJH" logo on two panels of the booth. autopano decided
these were a perfect match, and tried to overlap them. Needless to
say, everything went to hell until I figured this out and removed
these "bad" control points. That's when I decided to make the
adjacency template.
7) Forgot to fill in "missing" control points for overlapping pairs
that didn't have any. When shooting indoors I frequently find images
pairs that have no matches, e.g. two adjacent shots along a blank
wall or blank area of ceiling. This can result in an Optimizer run
that reports only 1 pixel of error, yet has an image that is
completely out of place. Here's where the "contrast enhancement"
trick comes in handy.
