You might want to try NoiseNinja, for those cases where you cannot shoot at
a lower ISO setting. This won't completely remove the grain, but does a
really good job with very little effort. It also can smooth out large shiny
surfaces even with low ISO images. I never realized the grain in some of
those shots until I tried applying NN to those images - sometimes it is even
useful at ISO 100, depending on the image contents.

They also supply "noise brush" settings to work on specific portions of the
image rather than the whole frame. The standard mode processes the whole
image, and is pretty good for most cases.
