D and E differences change everything in regards to abc.
You absolutely, must optimize everything together. Sometimes, abc first and
alone.. but generally, on a well positioned nodal point and well done
control points all you need is one pass of abcde all together.

If you have an slr, de should not change, if you have a pro-sumer, like a
Nikon coolpix or a Minolta a1 or example, DE should of course change by many
pixels. Just wobble your lens, that's DE, what makes matters, worse, is that
this will change from zenith to nadir shots.. and then you just don't bother
too much about it and stitch your best. 
The best method for DE is proper control points on all images, and last and
first image, and especially a zenith and nadir shot done on nodal point. 
This will really tighten the center of your distortions.
Just a cylindrical can have significant value differences if you use few
control points.

The various tutorials about optimizing for lens settings disagree on some
points. One thing that is still not clear to me: Is it better to do the d
and e optimizing with *only* d and/or e selected, or should yaw, pitch and
roll also be included in that optimizing step.?

I have saved as "template" a file where I first removed the control
points and that seems to work.

One thing is still not clear to me: The horizontal and vertical shift
parameters (d and e) are there to correct the off center position of
the lens and the sensor, right? So why would they change with each new
set of control points?
